Can I complete the new patient forms before my first visit?
Yes. Below is a link to download and print a new patient form. The completed form may be filled out in advance and handed to our office staff during your first visit.

Can my child come into the office without a parent?
Yes, established patients may present with a person listed on the HIPAA form. If your child is able to drive themselves, a Consent for Treatment of a Minor must be completed and sent with them.
